Pretty good recap, but a few questions:

Who announced - Johnny Olson or Gene Wood?

If Johnny then between September-early November; If Gene then November-February.

If Johnny, then what color was the border of Contestant's Row - Pink or Green?

Also if Johnny, which opening was used - The 'Prizes behind every Door' or 'Front Row pan'?

Just want to know.

The standard opening for Tom Kennedy's version was the prizes behind every door and then cut to the crowd for the First Four.

'It was pink.'

Then your episode is from either September or October. The green border was implemented by October 29th.
